Transition Individualized Education Plans (IEP)


When a student with an IEP turns 16 years of age, the IEP team will begin to develop a ‘transition plan’ as part of  school services.  This plan will describe the educational and functional steps to be taken to prepare the student for adult living.   The student's TCRC Service Coordinator is available to participate in the IEP meeting and help coordinate goals and services.  The team will determine which of the local classrooms would be most appropriate for the student.   Transition Classrooms are able to provide opportunities to help youth acquire necessary skills for adulthood and independence.  Educational services may be available until the student turns 22 years old.
