Service Coordinator II - Adults and Transition age


Our Atascadero team is looking for people who want to make a difference in their community and provide person-centered assistance to adults with developmental disabilities.

Service Coordinator IIs manage a caseload of 60-75 individuals who require specialized assistance.


We have openings on both the Adult team and the Transition team (working with individuals 15-22).


Candidates must have at least 2 years in social work and 1 year working with adults with developmental disabilities. Also required is a master's or bachelor's degree in human services. 


Primary Job Duties of Service Coordinators:

  • Support individuals we serve by maintaining regular contact and doing Individual Program Plans (IPP).
  • Participate in Interdisciplinary Team (IDT) staffings and attend meetings in the community as needed
  • Provide information and referral to generic services and resources in the community, including schools, recreational options and local support groups, supportive employment/ habilitation funded day programs, local agencies (C.C.S., Dept. Rehabilitation, Social Security, Medi-Cal, IHSS), specialist physicians/ specialists (speech therapist, O.T./P.T.).
  • Coordinate and secure TCRC funded services, including behavior services, training for independent living, supportive living, out-of-home placement, in home and out-of home respite, adult day programs, durable medical equipment and specialized services, non-Medi-Cal funded services.

Minimum Qualifications:

  • Master’s or Bachelor’s degree in human services field or a related area, with 2 years of applicable experience in mental health or social work; at least 1 year experience with adults  with developmental disabilities required. 2 years of case management experience is preferred.
  • Valid California driver’s license and reliable, insured vehicle or equivalent means of transportation.
  • Able to travel to any location at any time (this position will require local travel)

This is a non-exempt, full-time, union bargaining unit position, to be paid hourly. The starting salary range is:

Bachelor’s Degree -- $19.39 - $21.37

Master’s Degree – $22.45- $24.74


In addition to salary, mileage reimbursement is provided at the current IRS rate of $0.535 per mile, and 5% annual salary increases according to union payscales.
