Child Health and Disability Prevention Program (CHDP)
The Child Health and Disability Prevention Program (CHDP) provides well-child physicals for children through age 21. The complete examination includes hearing, vision, lead level, anemia check, urinalysis and immunizations. If the child is Medi-Cal eligible, services extend to age 21.

Other sources of these services include: local public education agencies, private preschools, public daycare, and the Head Start program. Information is available at www.dhcs.ca.gov
